Upgrading LFPSE from v5 to v6

The Learn From Patient Safety Events (LFPSE) taxonomy is part of the NHS England’s national system for recording and learning from patient safety events.
Its purpose is to record, categorise, and analyse patient safety events across all healthcare settings (primary, secondary, community care). The goal: better data → better learning → safer care.
Ideagen Healthcare Guardian supports LFPSE v6. This article outlines how to updated your existing v5 configuration to LFPSE v6.
Important Note: Before going live with V6, you will need to request approval from NHS England. If you try to submit to the V6 API before approval, you may get an error informing you that your organisation is not authorised.
Updating the LFPSE version
- Open Administrator and navigate to the “NHS Incidents – LFPSE” add-in.
Ensure the Questionnaire is set for LFPSE V5 and saved.
- Fill in details for LFPSE v6.
- LFPSE Token: This will be the same token as you use for v5.
- EndPoint URL: https://developer.learn-from-patient-safety-events.nhs.uk/
- Extension URL: https://developer.learn-from-patient-safety-events.nhs.uk/
- Click on the “Update to LFPSE V6”. This will show the list of changes. The changes will reset order of questions and may move questions to their original section to ensure the the conditional elements are correct.
Note: This list can be exported to Excel.
- Click the “Upgrade” and the LFPSE questionnaire will be updated and saved as DRAFT. As the questionnaire is in Draft, there will not be any changes applied to the live questionnaire.
Navigate to the LFPSE questionnaire and review the updates. The version picker will show “Draft”.
Updates can be made to the draft and saved in draft by using the “Save as draft” button.
- Once the questionnaire has been reviewed and approved, save as a “New Version”. This is required as all existing instances of LFPSE will stay as V5 and are not migrated. If “Save & Overwrite” is used, the application will return an error stating that a new version is required.
- New instances of LFPSE incidents will now use V6 and their updates to NHS England will use the V6 API.
