- You can access the data collection form using a QR code, a desktop icon or a URL
- The feedback form is designed to collect any type of feedback, not just complaints.
- You can also choose to have separate forms for different feedback types if you wish
- Our forms are device responsive and will adapt perfectly to your screen size
- Once you select the type of feedback you wish to record, more questions will appear that a relevant to your previous answers, so only relevant fields are filled in
- There is some information at the top of the form in case the user wishes to send in feedback via a letter, phone call or email instead
- As you move down the form, you’ll notice that some fields are marked with a red asterisk. These fields are mandatory, and you will not be able to submit the form without answering them
- At the bottom of the form, there is the ability to identify a privacy policy, so you can link a privacy policy to it so users always have access to it
- After you have completed all of the fields, you can click submit
- You will get a feedback reference number.
- At this point, you can close the browser
