1. Reporting an incident

There are a few different ways to access the incident form when you need to report an incident. You can:

• Scan the incident form QR code
• Click the incident form link embedded in your intranet or email signature

When you scan the code or click the link, you’ll be taken directly to the form to complete and submit your report.
