This video shows you how to access the Mortality app, how to submit a review, how to search for an existing review, and to view any actions that have been made against the review. 

To access the Mortality app:

1. Open Ideagen Healthcare Guardian Home 
2. Click on the Mortality icon on the Home Page. 

To submit a review:

1. Click on the 'Review' button.
2. The Mortality review form will open up in a new tab in your browser. 
3. You will need to fill out the relevant fields in the answer boxes provided. 
4. Some of the answer boxes are select boxes, some are free text boxes, and some are date pickers. 
5. Fields with a red asterisk are required fields. You won't be allowed to submit the review without filling out these fields. 
6. Some parts of the form allow you to repeat sections, such as adding the details of multiple consultants. 
7. Once you have completed everything, click 'submit.'

After submitting a review:

1. Any review that you have submitted, or have been named as the medical examiner, will appear on the review page. 
2. Buttons that run across the page display a count of how many reviews sit within each stage. 
3. You can see all open reviews by clicking on the 'Open Reviews' link in the menu bar. 
4. You can use the slicers at the top of the page to filter the information. 
5. Click the 'View' button to open the form and see the details for that review. 
6. Updates to the record can be made in the form, including creating actions for the review. 
7. Buttons along the bottom of the page allow you to move the review along different stages of the workflow, or save in it's current stage. 
8. If you need to look at closed, rejected or archived reviews, there is a page for this, too. 

To create an action:

1. Search for the review you would like to create an action for.
2. Open the form and click on the 'Action' icon in the action section of the form. 
3. Fill in the details. 
4. Click save. 
5. Ideagen Healthcare Guardian has a built-in action tracker which will display all actions that have been created against reviews. 
6. You can use the slicers at the top of the page to filter the information. 
7. To update any actions, click on the Data Entry icon in the top right-hand corner of the table. 

To search for a review:

1. Navigate to the search page. 
2. The search criteria run down the left-hand side of the page.
3. Enter the details of the record and click 'Search.'
4. Reviews that match your search criteria will appear, along with a count of the number of results returned. 
5. Click the 'view' button to view the details of that record.
